We use Spambayes (http://spambayes.sourceforge.net/) and are very satisfied
with the results.  Basically you train it to your taste in ham and spam.
Retraining is sometimes necessary when you notice more spam getting into
your INBOX, but this doesn't happen often for us.  Occasionally we have to
review our "unsure" spam directory for those emails on the cusp of being
spam and that's it.
